COVID-19 Vaccine Pre-Registration

COVID-19 vaccination planning has begun for those populations listed in Phase 1B. If you are aged 75 or older and live in Sarpy or Cass County, the Health Department is offering a pre-registration process for receiving additional information regarding COVID-19 vaccine availability.  This survey does not register you for a vaccine; it is a mechanism for you to be contacted once you are eligible to receive the vaccine.  COVID-19 Vaccine Pre-Registration Survey for Residents Aged 75 and Over

If you do not have access to a computer and would like to pre-register, please call 531-203-0322 and leave a voice message with your name, address, date of birth, phone number, as well as an email address if you have one.  A staff member will use this information to register for you.
